Outreach Program

The Outreach Program provides residential services to adults with chronic mental illnesses or physical disabilities in their homes.  Outreach is staffed to provide supportive services that enable the residents to function in the community and the residential setting with a high amount of independence.

Services provided:

  • Monitoring of, assistance with, and instruction for independent living skills (budgeting, banking, meal-planning, shopping, cooking, cleaning, hygiene, organization, leisure skills, coping skills, time management, problem-solving skills, safety skills, etc.)
  • Monitoring of and instruction on residents' rights and self determination.
  • Monitoring of health by observing and documenting changes in health and referring to health care providers when necessary.
  • Arranging, transporting, and accompanying to leisure and recreational activities.
  • Arranging, transporting, and accompanying to medical appointments.
  • Liaison with other service providers.
