Independent Care Health Plan (iCare) has entered into an innovative agreement with RCS Empowers, Inc. to provide staff to serve as HealthGuides to iCare members. This partnership provides iCare members with a familiar and trusted community resource along with access to enhanced care management services.
RCS HealthGuides assist iCare Care Coordinators (CC) to address the needs of iCare members in Sheboygan County, at times providing personal, face-to-face interaction between the member and iCare's Milwaukee-based care management teams. The goal of the HealthGuide program is to ensure that iCare and RCS are properly aligned to individual member needs and preferences, adjusting coordination and delivery of those conditions as they change over time. RCS HealthGuides function as a local presence in support of the efforts of iCare CCs in carrying out assessments, care planning, medical advocacy, post discharge follow-up, and other services.
Independent Care offers the iCare Medicare Advantage Special Needs Plan (SNP) - an insurance plan for individuals who are simultaneously eligible for both Medicare and Medicaid benefits; and the iCare Medicaid SSI plan - an insurance plan for individuals who are entitled to SSI or SSDI payments and full Medicare benefits. Individuals can be simultaneously enrolled in both iCare's SNP and its SSI plan.
