Supportive Home Care
The Supportive Home Care (SHC) Program provides residential services for adults
with disabilities in their home and in the community. The program is staffed up to
12 hours per day to provide supervision and supportive services that enable the
residents to function in the community to their optimum level of independence.
Services provided:
- Monitoring of, assistance with, and instruction for independent living skills (budgeting, banking, meal-planning, shopping, cooking, cleaning, hygiene, organization, leisure skills, coping skills, time management, problem-solving skills, safety skills, etc.).
- Monitoring of and instruction on residents' rights and self determination.
- Monitoring of health by observing and documenting changes in health and referring to health care providers when necessary.
- Medication monitoring.
- Assistance with meal preparation.
- Arranging, transporting, and accompanying to leisure and recreational activities. This may include group activities.
- Arranging, transporting, and accompanying to medical and other appointments.
- Liaison with other service providers.
